Some hospital stays involve multiple discussions, changes in care plans or ongoing preparations for discharge.
For families who would benefit from continued nursing involvement, Liora offers Transition packages:
Transition — 3 Hospital Encounters: $1,000
Three hospital-related nursing encounters to support continuity across a hospital stay, discharge or transition in care.
Transition — 5 Hospital Encounters: $1,600
Five hospital-related nursing encounters for families who need more ongoing support across these transitions.
Both packages have no expiry date. Unused encounters are subject to Liora's applicable service terms.
