Terms of service
1. Service Overview
This Agreement sets out the scope, purpose, limitations, and terms under which Liora Ministry™ of Health Design Innovation provides nurse-led continuity support services. Liora Ministry™ is a social program of Health Design Innovation.
Liora Ministry™ is a nurse-led continuity-of-care service. The purpose of Liora’s services is to support patients and families during key moments of transition or uncertainty in the healthcare journey by providing:

clinical clarity
-
communication and language support
-
continuity of understanding across settings
Liora works alongside existing healthcare providers and systems and does not replace medical, nursing, or publicly funded healthcare services.
The Client acknowledges and agrees that all services provided by Liora Ministry™ are delivered by regulated health professionals acting strictly within their individual scope of practice as defined by applicable Ontario legislation and professional standards.
Services are limited to assessment, education, support, care coordination, interpretation of health information, and escalation of concerns, and do not include medical diagnosis, prescribing, initiation or alteration of medical treatment, or the performance of any controlled act except where expressly authorized by law.
Liora Ministry™ does not replace, direct, or assume responsibility for the clinical decision-making of physician, nurse practitioner, midwife, other healthcare provider or emergency services.
2. Definitions
“Liora Ministry™‘s Personnel” and “Service Providers” means individuals engaged by Liora Ministry™ and or Health Design Innovation, who provide services under this agreement, strictly within their own professional scope of practice, credentials, and legal authority.
Nothing in this Agreement authorizes any Personnel to supervise, direct, or oversee the performance of controlled acts by another regulated or unregulated person except where expressly permitted by law.
3. Services
Liora provides time-limited, clearly defined services, which may include one or more of the following:
Clinical Assessment
Before participating in Clinical Navigation & Care Coordination services, Clients are required to complete a Clinical Assessment conducted by a Registered Nurse (RN) or Registered Practical Nurse (RPN).
This assessment allows the nurse to:
• review the Client’s current health status and relevant medical history
• understand the Client’s diagnoses, medications, and recent treatments
• identify key concerns, risks, or care priorities
• ensure that the nurse attending the appointment has sufficient clinical context to support the Client appropriately
• prepare focused clinical questions and goals for the upcoming medical visit
This assessment helps ensure that the navigation service is safe, efficient, and clinically meaningful, allowing the nurse to provide informed support and helping the healthcare visit address the Client’s most important concerns.
Clinical Navigation & Care Coordination
A professional nursing service designed to enhance continuity of care and support patients and families during medical appointments, outpatient procedures, or day surgeries. The goal of this service is to help the Client clearly understand their care plan, communicate effectively with healthcare providers, and navigate complex healthcare decisions.
Time-limited visit of healthcare professional support, which may include:
• preparing clinical questions and goals for the medical visit to ensure priorities are addressed
• conducting a nursing review of the Client’s current health status and medical history
• facilitating clear communication and clinical alignment between the Client and the healthcare team
• translating medical terminology and physician or nurse practitioner instructions into plain language (bilingual support available)
• synthesizing physician or nurse practitioner recommendations and next steps for the Client and family
• updating the Client’s personalized care plan and medication list based on new medical information
• providing professional nursing presence and oversight during outpatient procedures or day surgeries when appropriate
• identifying issues that may require further medical attention or follow-up coordination
• offering recommendations for community services and healthcare resources appropriate to the Client’s situation
Optional – spiritual support through prayer and connection with Christian community resources.
Service Limitations & Compliance
Following the visit, the Client will receive a summary of the discussion, which may be shared with the Client’s primary healthcare provider.
Liora Ministry™ is responsible only for services provided within the scope described in this Agreement and in accordance with applicable professional standards.
Liora Ministry™ is not responsible for outcomes arising from medical decisions, treatments, or services provided by third parties, nor for a Client’s failure to follow the advice of their primary healthcare provider.
Bilingual Support Disclaimer

Informal Nature:
The Liora Ministry™ team provides bilingual health advocacy as an informal support service. We are not a certified medical translation agency.
Accuracy:
While Liora Ministry™ strives for accuracy, medical language can be complex. The Client acknowledges that Liora Ministry™ is not responsible for any misunderstandings or errors arising from the translation of information provided by physicians or hospital staff.
Final Authority:
The official medical record provided by the healthcare provider (in its original language) remains the only legally binding record of the Client’s care.
Waiver of Responsibility for Client Valuables
Clients are responsible for the safekeeping of their personal valuables during hospital stays or clinic visits.
The Client acknowledges that valuables such as hearing aids, dentures, glasses, jewelry, or money should not be left unattended. If valuables are left unattended, the Client agrees that Liora Ministry™ is not responsible for any loss or damage.
Virtual Clinical Clarity Consultation
A scheduled phone or video consultation led by a healthcare professional to:

explain health information, test results, or care and/or treatment plans in plain language, and what to expect

review medications at a high-level for understanding only
-
teach proper medication usage and important precautions for side effects
-
help families prepare questions for healthcare providers
-
help the client understand the Canadian healthcare system
-
offer translation for communication needs.

identify issues that may require further medical attention
-
Provide recommendations for community services and medical resources when appropriate for your situation.

Optional – spiritual support through prayers and connecting with Christian community resources
Following the consultation, the Client will receive a summary of the discussion, which can be shared with the Client’s primary healthcare provider.
Liora Ministry™ is responsible only for services provided within the scope described in this Agreement and in accordance with applicable professional standards.
Liora Ministry™ is not responsible or liable for outcomes arising from medical decisions, treatments, or services provided by third parties, nor for a Client’s failure to follow the advice of their primary healthcare provider. Liora Ministry™ shall also not be liable for any outcomes resulting from a Client’s failure, refusal, or inability to follow, implement, or act upon the recommendations, guidance, or advice provided by Liora Ministry™. Once recommendations or guidance are communicated, the Client acknowledges that responsibility for implementation and any resulting outcomes rests solely with the Client.

Medication Education Limitation
Any review or discussion of medications is provided for general education and understanding only.
Liora Ministry™ does not prescribe medications, adjust dosages, discontinue medications, or provide medical instructions. All medication decisions remain the sole responsibility of the prescribing provider. Clients are advised to consult their prescribing clinician with any medication-related questions or concerns.
Hospital & Care Transition Support
Time-limited 2-hour visit of healthcare professional support during transition periods, which may include:

hospital admission or inpatient stay
-
discharge planning: help to create care and discharge plans
-
transition from hospital to long-term care or home
-
transition from long-term care or home to hospital
Support may include
-
in-person or virtual presence
-
clinical consultation, explanation, and recommendations to the Client of diagnoses, procedures, treatment and care plans, and what to expect during hospital stays
-
Working with the Client and the Client’s healthcare team to create personalized care plan
-
communication support with the primary healthcare provider or healthcare teams
participation in interdisciplinary rounds of patient or family meetings (subject to hospital policies and scheduling). -

-
Provide recommendations for community services and medical resources when appropriate for your situation.

Optional – spiritual support through prayers and connecting with Christian community resources
This service is provided for a defined episode and concludes at the end of that episode.
Liora Ministry™ is responsible only for services provided within the scope described in this Agreement and in accordance with applicable professional standards.
Liora Ministry™ is not responsible for outcomes arising from medical decisions, treatments, or services provided by third parties, nor for a Client’s failure to follow the advice of their primary healthcare provider.

WAIVER OF RESPONSIBILITY FOR CLIENT VALUABLES
The client acknowledges that any valuables should not be unattended during hospital stay and clinic visits. If client chose to keep valuables unattended, the client absolve the Service Provider of any liability for such valuables, such as hearing aids, dentures, glasses, jewelry, money, etc.
Liora Ministry™ Healthcare Team Home Visit
One-time or bundled healthcare professional check-ins to:

conduct an assessment for the purpose of identifying observed or reported signs, symptoms, risks, or concerns that may require further attention, education, or escalation to the appropriate healthcare provider identify early warning signs

assess safety and wellbeing
-
provide guidance on next steps
-
provide recommendations for community services and medical resources when appropriate for your situation.

Optional – spiritual support through prayers and connecting with Christian community resources
If urgent concerns are identified, Liora Ministry™’s Personnel will follow an established escalation protocol to notify the appropriate healthcare provider and authorized emergency.
Liora Ministry™ is responsible only for services provided within the scope described in this Agreement and in accordance with applicable professional standards.
Liora Ministry™ is not responsible for outcomes arising from medical decisions, treatments, or services provided by third parties, nor for a Client’s failure to follow the advice of their primary healthcare provider or the advice given by Liora Ministry’s™ service provider.

Following the Home Visit, the Client will receive a summary of the visit, which can be shared with the Client’s primary healthcare provider.
Home Visit Limitations
Home visits are non-medical in nature and are limited to nursing assessment, observation, education, safety review, and care coordination.
While Liora Ministry™ provides expert clinical guidance and assessments, Liora Ministry™ does not provide medical diagnoses or medical treatments that require a doctor or nurse practitioner’s order during home visits.
Liora Ministry™ Weekly Health Stewardship (Add-On Service)
Bundled professional check-ins to:

conduct a professional assessment for the purpose of identifying observed or reported signs, symptoms, risks, or concerns that may require further attention, education, or escalation to the appropriate healthcare provider identify early warning signs

assess safety and wellbeing

provide recommendation on next steps

provide recommendations for community services and medical resources when appropriate for your situation.

Optional – spiritual support through prayers and connecting with Christian community resources
If urgent concerns are identified, Liora Ministry’s™ Personnel will follow an established escalation protocol to notify the appropriate healthcare provider and authorized emergency contact.

Recommendations and Escalation
Any recommendations provided by Liora Ministry™ are limited to education, clarification, self-advocacy support, and guidance on when and how to seek further medical care.
Where concerns are identified that fall outside the scope of Liora Ministry™’s services, Liora Ministry™ will recommend escalation to, or communication with, the appropriate healthcare provider or emergency service.
4. Emergency Situations and Duty to Act
The Client acknowledges that Liora Ministry™ are bound by professional and ethical obligations to act in situations involving safety risk.
Where, in the reasonable professional judgment of Liora Ministry™, there is an imminent risk of serious bodily harm to the Client or others, Liora Ministry™ may take immediate action to support safety, including contacting emergency services (911) and/or notifying an authorized emergency contact, even if this is contrary to the Client’s expressed wishes at that time.
Actions taken to address imminent safety concerns are undertaken pursuant to professional and ethical obligations and do not constitute medical treatment, diagnosis, or the assumption of ongoing clinical responsibility beyond the scope of agreed Liora’s services .
5. Transportation
If the Service Provider is required to provide transportation in their own vehicle to carry out duties on the client’s behalf, the client will be charged mileage at a rate of $0.75/km, which shall be inclusive of automobile insurance and licensing, gasoline, tickets or citations for parking or driving offences, and maintenance and repairs. The Client will not be charged for mileage to the primary location where Services will be provided. In addition, the client agrees to reimburse Liora Ministry™ for any out-of-pocket expenses that the Service Provider has incurred that are directly related to such transportation services, if any, including but not limited to parking fees, tolls, and ferry fares. These charges will be noted on the bill and are payable as part of the client’s billing.
Transportation Waiver
If the Client’s vehicle is used for such activities requiring transportation, the Client will be solely responsible for ensuring that there is appropriate insurance coverage on such vehicle and ensure that the Service Provider who will perform the driving duties, with the Client and any such other occupants of such vehicle, will be fully covered by the Client’s insurance policy (proof of such coverage to be provided to Liora Ministry™ before driving commences).
Furthermore, the Client will assume all risk related to any driving duties performed by the Service Provider while in the Client’s car and accordingly indemnify and save harmless Liora Ministry and Health Design Innovation, including all Health Design Innovation’s Personnel, from any and all liabilities, losses, suits, claims, demands, costs, fines and actions of any kind or nature whatsoever (including attorney fees) to which Liora Ministry™ and Health Design Innovation and Health Design Innovation’s Personnel shall or may become liable for, or suffer arising from the transportation services performed in accordance herewith.
6. Food Purchase, Preparation, and Consumption Liability Waiver
The Client is responsible for informing Liora Ministry™, in advance of any food or beverage being purchased, prepared, delivered, or provided, of any dietary requirements, restrictions, allergies, sensitivities, medical conditions, religious or cultural considerations, or other limitations that may affect the safety or suitability of food or beverage consumption.
Liora Ministry™ relies on the accuracy and completeness of information provided by the Client and is not responsible for risks arising from incomplete, inaccurate, or undisclosed dietary information.
If the Client requests, or voluntarily accepts, food or beverages purchased, prepared, delivered, or otherwise provided by Liora Ministry™, the Client acknowledges and agrees that:
Responsibility for Choice and Consumption
The Client is solely responsible for selecting, approving, or voluntarily accepting all food and beverage items. Liora Ministry™ does not guarantee nutritional value, safety, allergen-free preparation, or compliance with any specific dietary requirements.
Waiver of Liability
The Client assumes all risks associated with the consumption of food or beverages purchased, prepared, delivered, provided, or offered by Liora Ministry™, whether requested in advance or voluntarily accepted, including but not limited to allergic reactions, foodborne illness, or digestive issues.
Indemnification
The Client agrees to indemnify and hold harmless Liora Ministry™ (including Health Design Innovation’s Personnel) from any and all liabilities, losses, suits, claims, demands, costs, fines, or actions of any kind or nature whatsoever (including legal fees) arising out of or related to the consumption of such food or beverages.
7. Financial Terms
The Client agrees to pay Liora Ministry™ for the services based on the prices and payment schedule. An invoice will be sent after the service has been provided. Payment is due upon receipt of the invoice.
Liora Ministry™ does not provide direct billing to insurance providers. The Client is solely responsible for paying all invoices to Liora Ministry™ and for submitting any claims for reimbursement to their own private insurance provider. While Liora Ministry™ provides documentation to support insurance claims, the Client remains responsible for all fees regardless of insurance coverage outcomes.
Payment Information
Liora Ministry™ assumes all responsibility with respect to the management of all remuneration for Service Providers. Clients’ payments for services are fully inclusive and clients will be charged no additional fees except as expressly set out in this Agreement. Liora Ministry™ (Health Design Innovation) has the absolute right, without limitation or penalty, to stop all services in the event of billing disputes and/or where delinquent charges are due to us. Agreement and Rate Changes
Liora Ministry™ may make changes to this Agreement upon giving the Client at least 14 days prior written notice. Notice may be provided to the Client at the most recent email address on file. Periodic assessments to determine the level of care and expertise required may result in a change of rate and/or Service Provider.
8. Privacy and Confidentiality
Liora Ministry™ will collect, use, and share the Client's personal health information in accordance with the Personal Health Information Protection Act, 2004 (PHIPA).
The Client authorizes Liora Ministry™ to communicate with healthcare providers and review relevant health information to support continuity, clarify, and escalate concerns. The Client authorizes Liora Ministry™ to release information and communicate on my behalf in accordance with the schedule in Schedule C.
The Client's information will be kept private and shared only with the Client's consent, unless the law requires otherwise.
The Client understands that Liora Ministry™ may be required by law to report certain problems, such as abuse or safety issues.
Liora Ministry™ will maintain the confidentiality and security of your personal health information in accordance with applicable laws and regulations. Liora Ministry™ will maintain clinical documentation in accordance with applicable professional regulatory standards and privacy legislation. Records will be accurate, objective, timely, and securely stored.
Records will be retained for the minimum period required by law and professional standards, after which they will be securely destroyed.
9. Electronic Communication and Informed Consent
The Client acknowledges that electronic communication (including email, text messaging, messaging platforms such as WhatsApp, WeChat and Telegram, and virtual platforms) carries inherent privacy and security risks. While reasonable safeguards are used, absolute confidentiality cannot be guaranteed.
The Client consents to the use of these methods for non-urgent communication and understands that they are not appropriate for emergencies or time-sensitive matters. In an emergency, the Client agrees to contact emergency services directly.
The Client may withdraw consent for electronic communication at any time by providing written notice.
10. Boundary of Spiritual Support and Prayer
Optional and Non-Clinical: Liora Ministry™ is a faith-based organization. While primary services are focused on clinical navigation and advocacy, spiritual support (such as prayer or scripture reading) is available as a form of personal comfort. Spiritual support is provided at no additional charge and is entirely separate from the billing for clinical navigation and advocacy services. It is agreed that:

Optional Nature: Spiritual support is strictly optional. The Client may request prayer at any time, and Service Providers may also offer such support.

Express Consent Requirement: Spiritual support shall only proceed if the Client expressly accepts an offer or initiates the request.

Quality of Care: A decision to decline spiritual support shall not influence the quality, frequency, or nature of the medical navigation and nursing advocacy services provided.

Non-Clinical Status: Spiritual support is provided for emotional and spiritual well-being; it does not constitute healthcare, psychotherapy, or clinical treatment, and shall not replace or influence professional medical or nursing decision-making.
11. Changes and Cancellations to Service
Liora Ministry™ will make every attempt to staff the appointment; however, in the event that a Service Provider is unable to staff the requested appointment due to unavoidable circumstances, Liora Ministry™ will notify the Client immediately.
If the Client no longer requires services, seven (7) days' written notice is required.

Services Already Provided: The Client will be billed for all work completed up to the time of notice.

Late Cancellation Fee: If less than 48 hours' notice is given for a scheduled visit, the Client will be charged the full cost of that specific visit (as the Service Provider cannot be re-booked on short notice).

Administrative Fee: For any other cancellations made with less than 7 days' notice, a flat Administrative Fee of $50 will apply to cover coordination and scheduling costs.
Should the Client wish to change Service Providers, this can be arranged by contacting Liora Ministry™ at connect@lioraministry.com or +1 (647) 848-3428.
The Client agrees to tell Liora Ministry™ about any changes to their health, plan of care, or schedule.
12. Termination of Services
Should Liora Ministry™ terminate the Client’s services, the Client will be given at least 7 days’ notice, and Liora Ministry™ will specify the reason(s) for the termination. Liora Ministry™ will advise the Client to follow with their primary care provider to ensure continuity of care, but cannot guarantee that any third party will accept the transfer.
The following exceptions shall apply:

Liora Ministry™ has the right to immediately terminate services if the Service Provider determines that there is a risk to safety. In this situation, a verbal notice will be given and followed up with a written notice including the reason for termination.

In the event of nonpayment for services rendered, Liora Ministry™ has the right to terminate services immediately as per the payment agreement described in this Agreement. Liora Ministry™ will transfer the responsibility for care to the Client’s primary care provider to ensure continuity of care.
Either party can end this Agreement by giving a minimum of 7 days' written notice.
Notice of Service Conclusion
Upon the completion of a care episode, Liora Ministry™ shall issue a formal Notice of Service Completion to the Client via electronic mail. The issuance of this notice, combined with the cessation of scheduled bookings, shall constitute mutual acknowledgement that the professional relationship for that specific episode has concluded. At such time, full clinical oversight and responsibility for the Client’s ongoing care shall transition back to the Client’s primary care provider.
To support the safe continuity of care, a clinical summary may be prepared. This summary is limited to information reasonably necessary for the transition, including service dates, observed clinical concerns, actions taken for escalation, and professional recommendations. The Client may elect to withhold consent for this information sharing by selecting the appropriate opt-out provision on the Notice of Service Completion.
In the absence of a client directive within 48 hours of service conclusion, Liora Ministry™ is authorized to provide a default safety notification to the Primary Care Provider to fulfill professional continuity of care obligations.
13. Gratuities and Gifts
Liora Ministry™ does not accept gratuities or gifts for services provided. In the spirit of the Ministry’s core values, please share positive feedback through Reviews. Liora Ministry™ loves to appreciate our Service Providers for the amazing work they do!
14. Infectious Disease Exclusion of Liability Clause
Infection Prevention and Control (IPAC)
Liora Ministry™ and Health Design Innovation’s Personnel will perform services in accordance with applicable Infection Prevention and Control (IPAC) practices, professional standards of practice, and public health guidance in effect at the time services are provided. This includes the use of appropriate precautions and professional judgment to reduce the risk of transmission of infectious or communicable diseases.
Inherent Risk of Community and In-Home Care
The Client acknowledges that the provision of services in home, community, hospital, long-term care, or other non-controlled environments involves inherent risks that cannot be entirely eliminated, including the risk of exposure to infectious or communicable diseases. Such risks may arise from factors beyond the control of Liora Ministry™, including but not limited to the presence of other individuals, environmental conditions, asymptomatic transmission, and limitations on information available at the time of service.
No Guarantee of Risk Elimination
While reasonable precautions will be taken, Liora Ministry™ cannot guarantee that exposure to infectious or communicable diseases will not occur. The Client understands that the absence of known symptoms or diagnoses does not eliminate the risk of transmission.
Assumption of Risk
The Client knowingly and voluntarily assumes the inherent risks associated with receiving services in community or in-home settings, including the risk of exposure to infectious or communicable diseases, except to the extent that such risk arises from a failure by Liora Ministry™ to meet applicable professional standards of practice.
Limitation of Liability
To the fullest extent permitted by law, Liora Ministry™ and Health Design Innovation shall not be liable for illness, injury, or loss arising from exposure to infectious or communicable diseases where such exposure occurs despite compliance with applicable IPAC practices, professional standards, and legal obligations.
Client Responsibilities
The Client agrees to disclose, to the best of their knowledge, any known or suspected infectious symptoms, diagnoses, or exposure risks prior to the provision of services, and to notify Liora Ministry™ promptly of any material changes in this regard.
15. Important Disclaimers
The services provided under this Agreement do not include medical diagnosis, treatment, or the dispensing of medication.
Liora Ministry™ is not responsible for outcomes of medical treatment, availability of external services, or decisions made by other healthcare providers.
The Client acknowledges that services provided by Liora Ministry™ are intended to support understanding, continuity, and navigation of care, and are not a substitute for medical advice or treatment. The Client agrees not to rely on Liora Ministry™ services as a replacement for care provided by a physician, nurse practitioner, midwife, other healthcare provider or emergency services.

16. Authorization for Release of Information
I hereby consent to and authorize Liora Ministry™ to release and receive information for the purposes of service provision, billing and payment, advocacy, interpretation, and education. The exchange of information may occur between, but is not limited to, physicians, third-party payers, other health care providers, and regulatory and/or accreditation reviewers. The Client’s confidentiality will be maintained at all times. This authorization is further detailed in a separate 'Authorization for Release of Information and Representation' form, to which the Client must review and agree. This separate Authorization form may be presented to healthcare organizations to facilitate the exchange of information as outlined in this agreement.
Without limiting the aforementioned, the Client hereby authorizes Liora Ministry™ to obtain and exchange information with the signed individual(s).
17. Notice
Any notice or communication under this Agreement may be provided by email and will be deemed sufficient if sent to the Client at the email address on file or to Liora Ministry™ at its designated email address.
18. Entire Agreement
This Agreement, including the pricing schedule, is the complete agreement between both parties and replaces all past agreements and understandings.
