We provide a 2-Hour Home Visit to check your health and make sure your home is a safe place for recovery.

What We Do During a Home Visit:

• Full Body Health Check: We do a professional health check. This includes checking your blood pressure and listening to your heart and lungs to see how your body is recovering.

• Hospital Instructions Follow-Up: If you also used our Hospital Transfer Support, we use this visit to follow up on the "Recovery Plan" the hospital gave you. We make sure every instruction from the doctors and nurses is being followed correctly so you stay safe at home.

• Wound & Surgery Check: If you have stitches or a wound from surgery, we check them carefully for any signs of redness or infection.

• Medication Safety Review: We look at the new instructions from the hospital and compare them with the medicines you already have at home. We help you organize them to catch any dangerous mistakes.

• Home Safety Check: We look at your home setup to make sure it is safe for you to move around. We want to prevent falls and make sure you have what you need to get better.

• Clear Next Steps: We explain your health information in simple English or Chinese. We help you know what signs to watch for and when you should call your doctor.

• 2-Hour Professional Home Visit: Dedicated time with a healthcare professional for a full assessment and recovery plan.

• Travel & Fees: Travel is Free within 20km of Queen’s Park. For visits between 20km and 40km, a flat $35 Service Fee applies to cover travel and time. Beyond 40km, please contact us for a custom quote. Parking is charged at the actual cost (receipt provided).
