Care Planning & Communication with Healthcare Teams
When medical information feels confusing and you're not sure what happens next, you need someone who can help you understand, organize, and communicate clearly with your care team.
You're Not Alone in Feeling This Way
Many families in Ontario face the same struggles. Appointments feel rushed. Doctors use words you don't understand. Test results arrive, but no one explains what they mean. Referrals get lost. Discharge instructions are unclear. You're left wondering: Did I miss something important? What am I supposed to do next?
These gaps in communication can lead to confusion, missed appointments, medication errors, and unnecessary stress. You deserve clarity, not more worry.
How I Help
I work with you to bring order and understanding to your care. As a registered nurse and patient advocate, I help you prepare for appointments, clarify care plans, communicate your concerns to your healthcare team, and organize next steps. I don't replace your doctors — I support you in understanding what they're saying and making sure your voice is heard.
You stay in control. I'm here to help you feel confident, informed, and supported as you navigate the Ontario healthcare system.
